If you are a patient registered at Reepham & Aylsham Medical Practice and move out of our catchment area, you will be required to register at a practice closer to your new address.
You will have 28 days after you move to register at another practice within your new area.
Once you have left our practice and registered elsewhere, we will no longer have access to your medical records as these will be transferred directly to your new practice.
